• Must be a United States citizen or have a green card
• Proof of age 18 or older
• Proof of High School Diploma or GED
• Two passport size color photos
• Three character references
• TB test
• Doctor's Statement that student is free of communicable diseases
• Arkansas State Board of Massage Therapy and Federation Board fee of $325.00
• These items will be required before graduation
